Client Forms: Required Prior to Hypnosis Appointments

These two forms are required 2 days before your session or your session will have to be rescheduled.

Please complete the two client forms (below) at least two days prior to your scheduled hypnosis session.  

Email your completed forms to Denise Deveney at DeniseDeveney@hotmail.com

Instructions: Download,  print both forms, fill in each page completely, sign, scan or take photos and email to Denise Deveney.

If you need a printer, check with your your local library for printing (some may even have a scanner). 

If you have any issues printing or returning the forms email Denise for issue resolution.
